Professional Development Fund Reimbursement Request

Fill out and submit this form to request partial reimbursement for any continuing education activity that benefits the MCS community as a whole. Examples include teacher trainings, rigging, CPR/First Aid trainings, nonprofit or fundraising conferences, or activities that provide an official certification, such as a personal training education program or a coaching certification program. Personal enrichment training does NOT qualify for reimbursement from this fund but may qualify for assistance from the general Scholarship Program.

To complete your application we need a receipt or proof of purchase clearly showing the total amount paid. Applications without receipts are not eligible for reimbursement. Please allow up to four weeks to receive reimbursement after your application has been approved.
