When any near-miss incident or injury requiring follow-up care from a medical professional occurs at MCS, please use this form to document what happened. Include the incident day and time, who was involved, and what activities were going on.
This form should be filled out by MCS members (for themselves or guests) or MCS instructors (for themselves or students). The member or instructor observing the incident should complete this form within 48 hours of the incident occurring.
